Rotations change, contracts shift, locum work fills the gaps, and private billings start before the paperwork catches up. Then EOFY lands right when your calendar is full and your practice setup is changing again, whether you are adding an associate in Northern Melbourne or stepping into ownership across Greater Brisbane.
The landmines are specific: payroll-tax reclassification risk for clinic contractor models, service-trust paperwork that fails the commercial-reality test, contractor-vs-employee classification mistakes, and HECS indexation timing that quietly compounds if no one plans for it.
We've built the practice around these problems. You don't have to teach us your industry - we already speak it.

If your practice distributes through a family trust, the 2026-27 Federal Budget proposed a 30% minimum tax on discretionary trusts from 1 July 2028. It isn't law yet and the details are still evolving, so we don't see a need to rush changes today. We're happy to talk through how it might affect you. Read our Budget 2026 trust distribution guide.

How the 2026 trust changes affect family distribution planning for practice owners.
Sale, succession, and ownership timing decisions that now need earlier planning.
A practical benchmark for specialist advice versus generic year-end-only service.
If your income is only salary, a generalist can be enough. Most doctors and dentists, though, run mixed income across PAYG, locum, private billings, service entities, or trusts. A specialist medical accountant helps you structure those streams correctly, claim within occupation-specific guidance, and avoid expensive compliance misses.
A service trust structure separates clinical income from administration and facility services. For many practice owners, that can improve tax outcomes and risk management when it is commercially real and documented properly. The setup has to match the ATO's commercial-reality expectations, including pricing, agreements, and actual conduct.
You might be, even if your doctors or dentists invoice as contractors. Recent state revenue activity has looked through agreements and focused on substance over labels. We review your contractor arrangements against current state payroll tax positions and map practical remediation options before a notice arrives.
Career stage matters. Registrar and locum arrangements often need a different setup from later specialist or owner stages. We start with clean separation of income streams, deduction evidence, HECS-HELP implications, and BAS obligations, then revisit structure as your private income mix changes.
Most clients move onto a fixed monthly package once we understand complexity and reporting needs. We scope migration, cleanup, and ongoing work upfront so there are no hourly surprises. You get a clear proposal before you commit, including what we take over from your current accountant.
